HAVE YOU BEEN DENIED?

• A dependent exemption, or
• Credit for a child under the earned income tax credit

Gather the following records and email or call us!

• Birth certificate(s) of your child(ren).
• Social Security card(s) of the child(ren).
• Statement from the child(ren)'s school, on school letterhead with the child(ren)’s full name and address. This statement should include the length of time the child(ren) lived at the address. Make sure a school official signs this statement.
• Statement from the child(ren)’s doctor, on office letterhead with the child(ren)’s full name and address. This statement should include the length of time the child(ren) lived at this address. Make sure a doctor or secretary signs the statement.
