Gather the following records and email or call us!

• Statement of Earnings from the Social Security Office for the tax years in question.  You can call toll free, 1-800-772-1213 and ask for form SSA-7004, earnings and benefit statement. This is a free service to the public.

• Any financial records from the tax years in question that support your claim as head of household (cancelled checks from mortgage payment, pay stubs and rent receipts, and utility bills in your name).
